Assess level of disability

In Armenia, a person's functional assessment is carried out through the "e-disability" electronic system. This system is operated by the Unified Social Service of the Ministry of Labor and Social Affairs of Armenia.

You can apply for this service in person by booking an appointment in advance.

To get a referral, the person or their legal representative must:

  •  
  • The person or their legal representative must apply to a medical organization for assistance.
  • The person must receive a referral for the functional assessment from their treating doctor (the doctor will provide the referral after conducting the necessary diagnostic, medical, and rehabilitation measures).

 

The doctor will indicate all personal and medical necessary information on the referral. The referral can be viewed through the "Armed" system (if the applicant's email address is registered in the "Armed" system, it can also be sent to the applicant via email).

 

The referral for the functional assessment is based on clinical data and results from laboratory and instrumental diagnostic tests. There is also a minimum list of necessary information, depending on health condition groups, which must be available at the time of referral.

The following steps must be followed after clicking the "Continue" button.

After receiving the referral, the following steps are required for the functional assessment:

  • Visit e-disability.am  (to view the English version use the browser's translation tool), 
  • Click on the "Application" button
  • Choose one of the four application types: primary assessment, re-assessment, re-evaluation, or death cause determination (in this case, primary assessment if it is the first application)
  • Select the application sub-type
  • Indicate the reason from the provided options
  • Choose who is submitting the application
  • Fill in personal information and the self-assessment form (the information must be filled in exclusively in Armenian)
  • Follow the system's instructions.


The applicant will be notified about the next steps of the application through the chosen notification method (in-person, by post, or by email).

Attention: 

  • The applicant's personal data remains confidential to the committee members and all participating specialists until the decision is made.
  • For applicants under 16 years old, the application must be completed by the legal representative of the person.

Requirements 

The presented documents must be valid within the last 30 days. Otherwise, after verifying the application, the system will notify that the submitted documents are incomplete.


Required Documents 

Public Services Number (social card) or a certificate stating the lack of a Public Services Number (HSH).


Delivery Period and Procedure 

  • The application is sent to the summarizing specialist.
  • The summarizing specialist checks the authenticity of the attached documents and provides the necessary clarifications required to assess the applicant.
  • A committee is automatically formed in the system to assess the decision based on the functional assessment.
  • The committee includes doctors, psychologists, occupational therapists, speech therapists, and other specialists as necessary.
  • Committee members are selected randomly from the registry of specialists involved in the functional assessment through the electronic system.
  • After this, the application is processed, and the status is assigned within 3 working days.
