How to apply

To register a child with a primary healthcare provider, after clicking the "Continue" button and undergoing identification, it is necessary to select the institution and the doctor.

Requirements

A child can be registered online with a primary healthcare provider by a parent who has a "YesEm" identification:

• one of the parents, in the case of a registered marriage,
• the mother, in the case of no registered marriage.

Required data

The child's birth must be registered prior to registering them with a primary healthcare provider.

Attention 
If the child's registration address and actual residential address are not the same, the service can be accessed physically by visiting the primary healthcare provider located in the area of the residential address (province).

Processing time and procedure 

• The registration of the child with a primary healthcare provider is done instantly.
• Within a few days after registration, the family doctor contacts the parent to arrange a home visit.
